With the premiere date finally announced for Person of Interest: Season 5, IGN can confirm that this will indeed be the final season for the ever-cool and compelling series.
Person of Interest's season premiere airs Tuesday, May 3rd at 10:00pm ET/PT. After the premiere, POI will air new episodes twice a week on both Mondays and Tuesdays at 10:00pm, beginning with a new episode on Monday, May 9th. Season 5 is comprised of thirteen episodes in total.
Said Person of Interest executive producers Jonathan Nolan and Greg Plageman, in a statement to IGN about the show's conclusion:

In key demographics, NCIS ranks in the top 10 among adults 25-54, averaging 4.6/13, and is in the top 15 in adults 18-49, averaging 3.1/10.
